I hereby give consent to train and play cricket for Kowloon Cricket Club and/or Cricket Hong Kong and participate in age-group and adult/open age cricket matches organised by Kowloon Cricket Club and/or Cricket Hong Kong, both in Hong Kong and overseas.

I acknowledge that cricket can be inherently dangerous and that serious accidents can happen, which may result in serious personal injury. I am fully aware and accept sole and unconditional responsibility for my own health and safety in the cricket environment, including the risk of personal injury, property damage and other losses and damage that I may incur while attending, exercising or participating in trainings and/or matches. I agree as a condition of my participating in the cricket environment, including but not limited to training and playing in matches organised by Kowloon Cricket Club and/or Cricket Hong Kong, that to the maximum extent permitted by law I absolve and shall hold Kowloon Cricket Club and Cricket Hong Kong harmless and fully indemnify them from all liability arising out of any injury to me howsoever caused in the cricket environment, including while training or playing cricket. I release and forever discharge Kowloon Cricket Club and/or Cricket Hong Kong and/or its coaches, instructors, agents, representatives, employees, partners of individually or otherwise from any or all claims, cause of actions, known or unknown, suits, actions, demands or right to compensation for damages that I may have in connection with my participating in the cricket environment. I authorise Kowloon Cricket Club and/or Cricket Hong Kong representatives to arrange medical or hospital treatment for me if I am unable to do so."

I understand that it is my full responsibility to make sure that I am in good physical condition and able to participate in trainings and/or matches and I confirm that I have been advised to consult a registered medical practitioner for an assessment on my physical fitness for the training program before enrollment

I further confirm that by agreeing to be enrolled in the training programme, I do so at my own risk, and I am voluntarily participate in these activities, assume all risk of any resulting injury.

Finally, I confirm that I fully understand that the legal implications of this Disclaimer and do hereby expressly accept and agree to be bound and abide by this Disclaimer in full out of my own wishes.

I authorise Kowloon Cricket Club and/or Cricket Hong Kong representatives to arrange medical or hospital treatment for my Child if I am unable to do so."

Health Declaration Form

Declaration:

I hereby declare and confirm that:

1. I do not have any of the following symptoms- fever, cough, sore throat, respiratory distress or other flu-like symptoms.
2. I have not been out of Hong Kong in the past 14 days.
3. In the past 14 days, I have not been in close contact with any person (e.g. family members, helpers etc.) who has been out of Hong Kong during the latter's 14-day quarantine period.
4. The guest have not been in close contact with any person who is confirmed or preliminary case of COVID-19 in the past 14 days.

The personal data provided will be used by Kowloon Cricket Club for the purpose of preventing the occurrence or spread of an infectious disease or contamination.

I undertake that the information provided above, to my best knowledge, is true, and understand that making a false declaration may jeopardize the health and safety of all staff and Members as well as the continued opening of the Club, and may render me to disciplinary action.

Important: Please note if you are not compliant with the above, you will not be given access to the Club.
